Waiver / liability release

Health and Safety:

The Member represents that they are in good health and have no condition that would prevent them from safely engaging in physical activities.
The Member agrees to inform the Gym of any changes in their health or medical condition.

Respect for Facilities:

The Member agrees to use the Gym's equipment and facilities in a safe and respectful manner and to report any damages or malfunctions immediately.

MK FITT Factory Cancellation Policy

We understand that life can be unpredictable, and sometimes you may need to cancel or reschedule your sessions. To ensure that we can accommodate all our clients and maintain the quality of our services, we have established the following cancellation policy for Group Fitness, Personal Training, and Massage Therapy sessions:

24 Hours or More Notice:

If you need to cancel or reschedule your session, please provide at least 24 hours' notice. This allows us to offer the time slot to another client.
There will be no charge for rescheduling or canceling your session with 24 hours or more notice.

12 to 24 Hours’ Notice:

If you cancel or reschedule your session with 12 to 24 hours' notice, you will be charged 50% of the session fee. This helps us cover a portion of the time reserved for you.

Within 12 Hours’ Notice:

Cancellations or rescheduling made within 12 hours of your scheduled session will incur the full session fee. This is because it becomes challenging to fill the time slot on short notice.

Waiver and Release of Liability:

The Member acknowledges that the use of Gym facilities and participation in fitness activities involves inherent risks. The Member agrees to release and hold harmless the Gym, its owners, employees, and agents from any and all liability for any injury, loss, or damage incurred by the Member.

Duty of Care Agreement for Parents/Guardians at MK FITT Factory

1. Responsibility for Supervision in the Kids Area
While at MK FITT Factory, I acknowledge that I am responsible for the supervision and behavior of my child(ren) at all times. I agree that my child(ren) will remain in the designated Kids Area provided by FITT Factory during my class or activity unless participating in an approved MK FITT Factory class for children.

2. No Unattended Children Outside Kids Area
Children are not permitted to wander outside of the Kids Area without adult supervision. I agree to ensure that my child(ren) remain(s) in the Kids Area or under my direct supervision while on the premises.

3. Use of the Kids Area
The Kids Area provided by FITT Factory is a designated space for children to stay safely during parent/guardian workouts. While this area is intended for the convenience of parents, MK FITT Factory does not provide formal supervision in the Kids Area. I understand and agree that I am responsible for my child(ren)'s behavior, safety, and well-being while they are in the Kids Area.

4. Facility Conduct
I agree to ensure that my child(ren) respect the equipment, staff, and other participants while at MK FITT Factory. Any damage caused to the facility or equipment by my child(ren) may result in liability for repair or replacement.

5. Fitness Floor Restrictions
For safety reasons, children are not permitted to access the fitness floor, equipment, or workout spaces unless they are participating in a designated children’s class with MK FITT Factory staff.

Acknowledgment & Agreement
I have read, understood, and agree to the terms of this Waiver and Duty of Care Agreement. I acknowledge that I am responsible for my child(ren)'s supervision and safety while at MK FITT Factory and within the designated Kids Area. I accept full responsibility for any consequences that arise from failure to supervise my child(ren) appropriately.
